Responsibilities

The Psychiatric/Mental Health LPN - Clinic is responsible for the collection and comparison of data used to conduct a focused assessment of the health status of a patient to normal findings and the individual's current health status in the clinics. Collaborates with providers and Registered Nurses to coordinate psychiatric services for patients experiencing psychiatric/mental health disorders in the clinic setting. Requires knowledge of nursing practices, which can be gained only through an accredited school of nursing. Oriented and educated in gender, cultural, and age appropriate care.

Hours for this role are Monday - Friday, 8a-430p

Responsibilities and Accountabilities:

    Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient's developmental age and to the patient's and family's diversity, preserving their autonomy, dignity, and rights.
  • Performs appropriate interventions as ordered by physician in an accurate and timely manner including needed testing such as lab draws, EKG, UDS, Genesight, and HcG testing. Documents results and provides safe handoffs to providers about the results.
  • Screens calls from patients and families, addresses the need and/or transfers the calls to a member of the clinic nursing support team based on their scope of practice.
  • Tracks and communicates pertinent medical and psychiatric information both verbally and through written documentation in EHR.
  • Maintains confidentiality in matters related to patients and families.
  • Provides care management for patients in person and over the phone utilizing psychosocial, physical, and behavioral data gathering related to specific conditions and collaborates with RN/Provider to provide individualized directives based on this data.
  • Adheres to safe infection control practices including good hand hygiene, standard precautions, cleaning equipment, and teach patients practices such as washing their heands and covering their cough.
  • Maintains safe, secure, and healthy work environment by adhering to operating guidelines, policies, and procedures.
  • Completes medication refills in an accurate manner consistent with policy and good nursing practice including verifying allergies and/or contraindications and monitoring for side effects. Administers long acting injections in a safe manner using safe nursing practices and consistent with clinic protocol. Educates patients and families about medications.
  • Obtains insurance pre-authorizations for medications and tracks prior authorizations.
  • Assists in completing and coordinating referrals.
  • Oriented and educated in gender, cultural differences and age appropriate care.
  • Monitors and addresses requests received in the task management system, assigns tasks to team members based on scope of practice.
  • Communicates effectively with interdisciplinary team members. Collaborates with team, seeks timely advice, and gives and always accepts feedback to ensure safe patient care.
Qualifications
  • Graduate of an accredited school of nursing.
  • Current licensure as an LPN with the Minnesota State Board of Nursing is required.
  • Evidence of 1 year of psychiatric/mental health nursing experience within the past two years preferred.
  • Must have critical thinking skills and ability to problem solve.
  • Ability to communicate and make safe judgment calls in stressful situations.
  • Ability to intervene in pre-crisis or crisis situations by utilizing de-escalation techniques, including developing a plan of safety for patients as needed.
  • Ability to multi-task, use time efficiently, and prioritize appropriately.
  • Ability to communicate in a professional manner in both verbal and written format and model a positive teamwork attitude.
  • Interpersonal skills-the individual maintains confidentiality, remains open to others' ideas and exhibits willingness to try new things.
  • Must have basic computer skills using Microsoft Office products and EHR.
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergent situations.
  • Looks for ways to improve and promote quality and demonstrates accuracy and thoroughness.
